Insider Profile – Amend or Remove Erroneous Ceased to be an Insider Date

Caution

If the insider has amended their profile to include a current relationship with the issuer, you cannot remove the ceased date from a previous relationship with that same issuer.

Removing an erroneously reported ceased date can only be done within 90 days.

How to Amend or Remove Erroneous Ceased to be an Insider Date

Steps to amend or remove an erroneously reported ceased date follow:

Step	Action
1	Click Insider profile in the top navigation bar
2	The Introduction to insider profile activities (Form 55-102F1) screen opens Click Amend insider profile in the left navigation bar The Amend insider profile screen opens Choose Option 2 and click Add or amend issuer information
3	The Add or amend issuer information screen opens Click the radio button beside the issuer to select it Click Amend The Amend issuer information screen opens Enter the correct date in the Date the insider ceased being an insider of this issuer field or click Reset to remove the date entirely and reinstate the relationship to the issuer as it was at the time the ceased date was reported. Note: You cannot modify the date the insider ceased to be an insider after 90 days from the date previously provided. Click Next
4	The File Amended Information screen opens Review amended information Click Certify The Certification dialog box appears Click OK
5	The Completed screen opens If you would like a printed version with a date and time stamp, click Printer friendly version. SEDI will display the profile in a format that is easily printed, and your browser’s Print dialog box will open. Once your browser’s print dialog box opens, print and click Back. Either click Yes to make another amendment, click No to return to the Introduction to insider profile activities (Form 55-102F1) page, or click Logout in the top navigation bar
