Filing Process for Dispositions Due to Bankruptcy

You report dispositions of securities as a result of bankruptcy proceedings in the same manner as you would other dispositions, except:

• Nature of transaction: Other - code 97
• General remarks: Disclose that the disposal is as a result of bankruptcy proceedings (maximum number of characters for remarks: 254)
• Date of transaction: Effective date of bankruptcy.

Instructions for Filing an Insider Report

Instructions to file an insider report for a disposition of shares due to bankruptcy is similar to filing a common share transaction or a stock option transaction with the exceptions noted above.
